    When I checked in I was given a form asking other health concerns that I wanted to discuss. This was either not given to the care provider or was ignored. Appointment felt rushed.

    Is efficient & knowledgeable and showed empathy when my 45 yr. old son died suddenly 2 years ago.

    Care provider didn't ask open end questions - only see him once a year, needs to access where I am at.
