An appointment with your personal physician should always be your first choice, but if your doctor is not available, we offer the following resources for treating non-emergency conditions that cannot wait until your physician’s office is open. The Medicenter costs the same co-pay as for a doctor's visit, and provides medical care to patients on a walk-in basis, and treat everything from the flu to headaches, from minor cuts and abrasions to athletic strains and sprains.

Advance Care Planning
Talking with your family, friends and your physician about your wishes for medical care at the end of your life is called advance care planning. It's a way for you, your loved ones and your physician to discuss the kinds of care you want and don't want at that time. You can also specify the care you would want if you become unable to speak or make decisions for yourself, due to a coma or other medical condition. When you write down your wishes, this kind of plan is called an advance directive.
An Advance Health Care Directive is a legal document that tells your physician, your family members and friends about what kind of care you would like to have if you become unable to make medical decisions. It's called an advance directive because you choose your medical care before you become seriously ill. To download a copy of an Advance Health Care Directive, click here.
Another form of advance care planning is Physician Orders for Life-Sustaining Treatment (POLST). POLST is a doctor’s order that indicates what types of life-sustaining treatment you do or do not want if you become seriously ill. Completing a POLST is voluntary, and does not replace the Advance Health Care Directive. To download a copy of the POLST form, click here.
The Advance Directive largely identifies a surrogate decision-maker should you become unable to make decisions about your health care. The POLST is intended to help you and your doctor discuss and develop specific plans to reflect your wishes. It also helps doctors, other health professionals and emergency personnel honor your wishes for life-sustaining treatment, especially in an emergency. The POLST is designed to be in addition to an Advance Directive.
Appointments
Appointments should be scheduled directly with your physician’s office. Please remember to bring your health identification card and other personal health records. These may include copies of recent tests or documentation from another doctor’s office.
We know missed appointments are often unavoidable. If you are unable to make your scheduled appointment, please call your doctor’s office at least 24 hours in advance. When you call, we will be happy to reschedule your appointment for a more convenient time.
In the event of a last-minute cancellation, we still appreciate your call so that we are able to accommodate other patients.

Billing
Most insurance plans require a co-payment, which is due at the time of service. After your appointment, your insurance will be processed and you will be billed for any services not covered by your insurance plan. If you are uncertain whether a particular service is covered, please contact your insurance company directly. If you have questions call St. Jude Heritage Customer Service at (800) 627-8106.
Care Management
Our experienced social workers and care managers are available to assist St. Jude Heritage patients and their families manage the medical, psychological and practical issues created by illness, hospitalization or medical treatment.
Registered nurse care managers and social workers offer a wide range of services, including:

care coordination for those with complex or catastrophic conditions, including coordinating appointments and services between specialties, as well as out-of-network services |

psychosocial evaluations and treatment planning |
| • |
assistance with arranging alternate levels of care for patients leaving the hospital, including rehabilitation facilities, skilled nursing facilities, hospice or other options |
| • |
help coordinating insurance benefits and answer questions related to MediCal, Medicare, Social Security, managed care and state disability |
| • |
assistance with arranging home health care, home hospice and the use of durable medical equipment |
| • |
counseling and referrals for the treatment of depression, substance abuse and other behavioral health issues |
| • |
connect patients with support and educational groups as well as other community resources |
| • |
life planning for seniors, including end-of-life issues and assistance with senior living options |

Changing Your Doctor

Electronic Medical Records
St. Jude Heritage Medical Group is leading the nation with the implementation of an electronic medical record (EMR). Charts no longer have to be filed, retrieved and transported. Now with a click, your complete medical history is available to any healthcare provider, so whether it’s your primary care doctor, a specialist or after hours care physician—your medical history is available, ensuring the healthcare provider has the information needed to provide an accurate diagnosis and treatment.
The electronic medical record (EMR) is critical to St. Jude Heritage Medical Group’s ability to provide efficient, coordinated, safe and high-quality care. With EMR any St. Jude Heritage healthcare provider can quickly and easily review test results and past exams or procedures. The record also provides important information about medication(s), including the dosage and time for renewal, drug interactions and a complete record of your immunizations and allergies.
The EMR also helps the staff quickly access a patient's medical record and reference the information needed to schedule appointments, answer phone calls and for prescription refills.
