Through a carefully designed blend of exercise therapy, education, and support we can help you recover faster, reduce your risk of future heart problems, and give you more energy to do the things you enjoy. Many of our patients feel better after completing our program than they did before their heart attack.
The multidisciplinary rehabilitation team includes a physician, psychologist, advance practice cardiac nurses, exercise physiologists, educators and registered dietitians—each contributing their expertise to ensure your successful recovery.
The program takes patients, who meet the criteria, through every stage of recovery. In the beginning, individualized cardiac-monitored exercise sessions are combined with ongoing education to learn heart healthy eating, stress management, and lifestyle changes. For those taking Coumadin or other blood thinners, frequent monitoring to measure the medication’s effect is essential. The St. Jude Heritage Medical Group Coumadin Clinic offers an easy and convenient solution: patients make an appointment, are seen promptly, have their blood tested (a finger stick puncture) and receive on-the-spot results. If your dosage needs to be adjusted, our clinical staff, who specialize in anticoagulation therapy, will discuss the change “face to face"—no more waiting days for lab results delivered via a phone call. Equally important, this cardiologist-supervised clinic offers personalized education about Coumadin, tailored to address specific needs and questions.
Patients are referred to the Coumadin Clinic by their St. Jude Heritage physician. Because of our state-of-the-art electronic medical records, your medical history and health information is instantly available whenever you are seen—and updated with each test result.
